Josephine Mporokoso, is both a senior clinical case manager and co-founder of Serengeti Care. Ms. Mporokos is a dynamic nursing professional with expertise in managed care, transitional care planning and geriatric patient populations. She holds both a Bachelors of Science in Nursing (BSN) and a Masters of Science in Nursing (MSN) from Western Governors University. Ms. Mporokos’ academic achievements, professional experience and commitment to excellence has made her an exceptional leader at Serengeti.

In her current role as senior clinical case manager at Serengeti Care, Ms. Mporokoso focuses on comprehensive nursing assessments, care planning and providing hands-on nursing care to meet the needs of her clients. She collaborates with multidisciplinary teams and provides ongoing education to certified nursing assistants. Ms. Mporokoso also plays a key role in developing training curriculum and facilitating communication between leadership and frontline staff. She conducts Continuous Quality Improvement (CQI) activities to improve care outcomes.

Ms. Mporokoso’s previous experience includes working as a nursing manager in an assisted living facility, where she supervised staff and provided direct patient care. She also served as a floor nurse in a skilled nursing and rehabilitation center, managed all direct nursing care, collaborated with interdisciplinary teams and provided education to patients and families.

Ms. Mporokoso’s skills include chronic disease process education, care planning, medication administration audits, patient/family education, and palliative/hospice care. She is proficient in professional relationship building, multidisciplinary care coordination, conflict resolution, mental health/behavioral health management and quality improvement processes.
