We make the VA process easier to understand and navigate. Here’s how it works:
Schedule an appointment with your Primary Care physician and ask about utilizing VA home care hours.
Your physician will assess your needs and submit a referral to the VA indicating the recommended number of care hours.
The VA will conduct their own assessment in the Veteran’s home to determine eligibility and level of care.
Once approved, the VA selects an authorized home care agency and sends the referral directly to that provider.
Once the VA referral is approved and sent, Serengeti Care can begin services quickly, ensuring continuity and peace of mind for Veterans and their families.
